The High-Fidelity Wraparound (HFW) Case Manager (CM) safeguards that the principles of the wraparound process are delivered with the highest possible fidelity to the model. Hours are Monday - Friday 8:00am - 5:00pm. Located in the Salt River Trail Region, KY, covering multiple counties. Requires at least 30% travel within and possibly outside the region. The HFW CM offers consultation and education to providers about wraparound process values, monitors progress toward goals, gathers and enters evaluation data, helps youth and families develop and integrate natural support systems, and manage their services and plans. Duties include completing comprehensive strengths-based assessments, partnering with team members to develop and implement service plans, facilitating team meetings, conducting home visits, monitoring services, identifying high-risk factors, coordinating care plan activities, consulting with case managers and health programs to overcome barriers, and presenting cases at conferences for multidisciplinary review. Attributes sought include experience building relationships with families, knowledge of community resources, experience facilitating family teams, engaging youth and families in care plans, and problem-solving with diverse teams. Required qualifications include residency in specified counties, willingness to travel 30%, 1+ years experience in Total Case Management for children/youth with Severe Emotional Disturbance, knowledge of Foster Care, juvenile justice, Behavioral Health Services, Trauma-informed Care, ACEs, Crisis Intervention, and evidence-based practices for Kentucky SKY populations, and certification or ability to complete training as HFW facilitator within 12 months. Preferred qualifications include respect for families with complex behavioral health needs and commitment to empowerment. Education required is a Bachelor's Degree in Human Services or related field.
